	Has your drinking or drug use caused any legal difficulties (DUI; domestic violence; public drunkenness; possession; curfew violations, etc)?

	Have you ever tried to stop using alcohol or drugs?
	Has anyone close to you expressed concern about your alcohol or drug use?
	Do you find it difficult to have a good time without using alcohol or drugs?
	Do you spend a lot of time thinking about ways to get alcohol or drugs?

*Answering yes to any of the above questions may indicate that you have a problem with alcohol and drugs that requires professional intervention.
