
Programs & Services : Recovery Care Management
Recovery Care Management is a recovery model that provides focused support, accountability and coordination of services to individuals and families in the achievement of their long-term recovery goals.
Chemical dependency is a chronic, progressive disease, which negatively impacts the physical, mental, emotional, social and spiritual aspects of an individuals life. Like any other chronic illness, there is always the risk of relapse.
As part of Carons continuum of care, Recovery Care Management seeks to minimize the risk of relapse and identify it in the early stages, by keeping clients engaged in the recovery process.
Services:
- An individualized strategic recovery plan with emphasis on personal responsibility and accountability
- Ongoing monitoring and support by a Care Manager, which includes phone contact, as well as face-to-face meetings with the client and their family
- Care Manager consultation with the Caron treatment team
- Support for crisis situations, including interventions
- Guidance in integrating into 12-Step recovery programs, including meetings, temporary sponsorship, etc.
- Guidance in integrating into therapy, aftercare and treatment programs
- Guidance in integrating into additional services as needed, i.e. vocational, legal, employment, etc.
Other Services Offered:
- Within the first six months, return to Caron for a relapse prevention checkup
- At one year, a revisit and/or participation in Carons Codependency Workshop
- Participation in two Caron Life Enrichment Workshops
Caron understands that the recovery process can incur obstacles along the way. Recovery Care Management brings the chaos of addiction and early recovery into a focused recovery plan.
